La progresi\u00f3n de la enfermedad depende de m\u00faltiples factores.&nbsp;&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">El&nbsp;<strong>colesterol<\/strong>, la&nbsp;<strong>tensi\u00f3n arterial<\/strong>&nbsp;(TA) y el&nbsp;\u200b\u200b<strong>accidente cerebrovascular<\/strong>&nbsp;(ACV) se encuentran entre los factores de riesgo cardiovasculares m\u00e1s&nbsp;importantes, que deber\u00edan estudiarse en profundidad. La presencia de factores de riesgo compartidos entre las enfermedades cardiovasculares y la EA podr\u00eda estar detr\u00e1s de la asociaci\u00f3n entre ambas enfermedades; no obstante, tambi\u00e9n podr\u00eda existir una relaci\u00f3n causal directa, ya que la cardiopat\u00eda provoca hipoperfusi\u00f3n y&nbsp;micro\u00e9mbolos, mecanismos implicados en la etiolog\u00eda de la EA. Adem\u00e1s, se ha demostrado la coexistencia de lesiones anatomopatol\u00f3gicas t\u00edpicas de la EA y de lesiones cardiovasculares en autopsias.&nbsp;<\/p>\n\n\n\n<p class=\"has-text-align-left borde-citas has-text-color has-background has-link-color wp-elements-ddf318fdb1b541ca405978d65aefa3fd wp-block-paragraph\" style=\"color:#1b4298;background-color:#fff8cc;font-size:18px;font-style:normal;font-weight:600\">El objetivo de esta revisi\u00f3n fue determinar y analizar ciertos factores de riesgo&nbsp;(colesterol, TA y ACV)&nbsp;asociados a la EA.&nbsp;<\/p>\n\n\n\n<div style=\"height:40px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading has-black-color has-text-color has-link-color wp-elements-c9531f6169c94b4e4dfd8f5400a8f957\" style=\"font-size:27px;font-style:normal;font-weight:700\">M\u00e9todos&nbsp;<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Este meta-metan\u00e1lisis&nbsp;se llev\u00f3 a cabo de acuerdo con la declaraci\u00f3n PRISMA y la b\u00fasqueda se centr\u00f3 en art\u00edculos publicados entre&nbsp;<strong>2020&nbsp;<\/strong>y<strong>&nbsp;2023<\/strong>.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Se incluyeron estudios que cumpl\u00edan los&nbsp;<strong>siguientes criterios<\/strong>:&nbsp;metan\u00e1lisis&nbsp;que investigaran el efecto del colesterol, la TA y el ACV (en la evaluaci\u00f3n basal) como antecedentes de&nbsp;EA (en el seguimiento); pacientes con diagn\u00f3stico de EA en base a criterios establecidos y&nbsp;metan\u00e1lisis&nbsp;originales, revisados por pares y publicados en ingl\u00e9s.&nbsp;<\/p>\n\n\n\n<div style=\"height:40px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading has-black-color has-text-color has-link-color wp-elements-94bef836019455abdd340eb722c90763\" style=\"font-size:27px;font-style:normal;font-weight:700\">Resultados y discusi\u00f3n&nbsp;<\/h2>\n\n\n\n<h3 class=\"wp-block-heading has-black-color has-text-color has-link-color wp-elements-f48bc381127950c1748f9ed0e8bf9f6d\" style=\"font-size:22px\">Colesterol y enfermedad de Alzheimer&nbsp;<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Se analizaron&nbsp;5&nbsp;metaan\u00e1lisis, de los cuales se seleccionaron 100 estudios primarios transversales y longitudinales,&nbsp;con 2\u202f289\u202f511 participantes&nbsp;y 12 magnitudes del efecto.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">El&nbsp;<strong>colesterol de&nbsp;las&nbsp;lipoprote\u00ednas de baja densidad (LDL-C) elevado<\/strong>&nbsp;constituy\u00f3 un&nbsp;<strong>factor de riesgo independiente<\/strong>&nbsp;para el desarrollo de EA&nbsp;(<em>odds&nbsp;ratio<\/em>&nbsp;[OR] = 2,55).&nbsp;&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">En cambio, no se observaron diferencias en las concentraciones s\u00e9ricas de<strong>&nbsp;colesterol de<\/strong>&nbsp;<strong>lipoprote\u00ednas de alta densidad<\/strong>&nbsp;(<strong>HDL-C<\/strong>) entre individuos sanos y pacientes con EA, ni una asociaci\u00f3n positiva entre las concentraciones de&nbsp;<strong>triglic\u00e9ridos<\/strong>&nbsp;(\u200b\u200b<strong>TG<\/strong>) ni&nbsp;<strong>colesterol total<\/strong>&nbsp;(<strong>TC<\/strong>) y el desarrollo de la enfermedad.&nbsp;<\/p>\n\n\n\n<p class=\"has-text-align-left borde-citas has-text-color has-background has-link-color wp-elements-61d1938e4b4ca0e7247f10600f598999 wp-block-paragraph\" style=\"color:#1b4298;background-color:#fff8cc;font-size:18px;font-style:normal;font-weight:600\">En conjunto, estos resultados indican&nbsp;<strong>una asociaci\u00f3n espec\u00edfica entre el LDL-C&nbsp;y la EA<\/strong>, mientras que el&nbsp;<strong>resto<\/strong>&nbsp;de&nbsp;las&nbsp;fracciones lip\u00eddicas&nbsp;<strong>no mostraron una relaci\u00f3n significativa<\/strong>.&nbsp;<\/p>\n\n\n\n<div style=\"height:20px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h3 class=\"wp-block-heading has-black-color has-text-color has-link-color wp-elements-6856e7fc1e277e11e4362c39662642ef\" style=\"font-size:22px\">Tensi\u00f3n arterial y enfermedad de Alzheimer&nbsp;<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Para este an\u00e1lisis, se&nbsp;incluyeron&nbsp;25 metaan\u00e1lisis, de los que se extrajeron 52 estudios primarios, tanto transversales como longitudinales, y 75 magnitudes del efecto. El tama\u00f1o total de la muestra fue de 1\u202f485\u202f392 participantes.&nbsp;<\/p>\n\n\n\n<p class=\"has-text-align-left borde-citas has-text-color has-background has-link-color wp-elements-5f4157ca9cff708bc1e1a7ea3a99d772 wp-block-paragraph\" style=\"color:#1b4298;background-color:#fff8cc;font-size:18px;font-style:normal;font-weight:600\">La&nbsp;<strong>hipertensi\u00f3n arterial<\/strong>&nbsp;(<strong>HTA<\/strong>) se asoci\u00f3 con<strong>&nbsp;mayor riesgo de EA<\/strong>.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">La presi\u00f3n arterial sist\u00f3lica (PAS) elevada mostr\u00f3 una asociaci\u00f3n estad\u00edsticamente significativa con la EA (OR = 1,09), aunque de magnitud modesta. En cambio, la hipertensi\u00f3n combinada (PAS y PAD [presi\u00f3n arterial diast\u00f3lica]) elevada no se asoci\u00f3 con un aumento del riesgo.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u200b\u200b\u200bEl&nbsp;an\u00e1lisis de&nbsp;los factores&nbsp;moderadores&nbsp;indic\u00f3:&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>diferencias por sexo \u00fanicamente en la relaci\u00f3n entre PAD elevada y EA, con&nbsp;<strong>mayor riesgo&nbsp;de EA (86\u202f%)&nbsp;en&nbsp;las&nbsp;mujeres con PAD &gt;90&nbsp;mmHg<\/strong>;&nbsp;<\/li>\n\n\n\n<li><strong>no hubo influencia de la edad de inicio<\/strong>&nbsp;(\u226465 frente a \u226565 a\u00f1os) y el dise\u00f1o del estudio;&nbsp;<\/li>\n\n\n\n<li>mayor riesgo&nbsp;de EA en personas hipertensas de&nbsp;<strong>Asia y Norteam\u00e9rica<\/strong>, pero no de Europa.&nbsp;<\/li>\n<\/ul>\n\n\n\n<div style=\"height:20px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h3 class=\"wp-block-heading has-black-color has-text-color has-link-color wp-elements-e6aaf759ea888862c8bc587e94b3f944\" style=\"font-size:22px\">Accidente cerebrovascular y enfermedad de Alzheimer&nbsp;<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Para este an\u00e1lisis, se incluyeron&nbsp;3&nbsp;metaan\u00e1lisis, de los que se extrajeron&nbsp;13&nbsp;estudios primarios&nbsp;(16\u202f547 participantes)&nbsp;y&nbsp;14&nbsp;magnitudes del efecto.&nbsp;&nbsp;<\/p>\n\n\n\n<p class=\"has-text-align-left borde-citas has-text-color has-background has-link-color wp-elements-f9ce58cd8c0c4998f894e2ae6705993f wp-block-paragraph\" style=\"color:#1b4298;background-color:#fff8cc;font-size:18px;font-style:normal;font-weight:600\">Se encontr\u00f3 una&nbsp;<strong>asociaci\u00f3n significativa entre el ACV y un mayor riesgo de desarrollar EA<\/strong>.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">No se observaron diferencias seg\u00fan el tipo de ACV:&nbsp;<strong>isqu\u00e9mico, hemorr\u00e1gico o&nbsp;microinfarto<\/strong>.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u200b\u200b\u200bEl&nbsp;an\u00e1lisis de&nbsp;los factores&nbsp;moderadores&nbsp;<strong>no&nbsp;mostr\u00f3&nbsp;diferencias seg\u00fan sexo, edad o regi\u00f3n geogr\u00e1fica<\/strong>.&nbsp;<\/p>\n\n\n\n<div style=\"height:20px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h3 class=\"wp-block-heading has-black-color has-text-color has-link-color wp-elements-1e21437cd923dbbd896f95bc3f08b706\" style=\"font-size:22px\">Todos los factores de riesgo y enfermedad de Alzheimer&nbsp;<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Al agrupar todos los efectos extra\u00eddos de los&nbsp;metan\u00e1lisis&nbsp;realizados&nbsp;con los diferentes factores de riesgo,&nbsp;la<strong>&nbsp;mayor&nbsp;magnitud&nbsp;del efecto<\/strong>&nbsp;se&nbsp;observ\u00f3&nbsp;para el ACV (OR = 2,27)&nbsp;(sobre todo, para&nbsp;la relaci\u00f3n entre los&nbsp;microinfartos&nbsp;y la EA&nbsp;[OR = 4,41]), seguido de la asociaci\u00f3n entre el LDL-C y la EA (OR = 2,55) y entre el ACV isqu\u00e9mico y la EA (OR = 2,12).&nbsp;<\/p>\n\n\n\n<p class=\"has-text-align-left borde-citas has-text-color has-background has-link-color wp-elements-a3b443ad12d40b72698b94ad28361881 wp-block-paragraph\" style=\"color:#1b4298;background-color:#fff8cc;font-size:18px;font-style:normal;font-weight:600\">Estos datos sugieren que los eventos cerebrovasculares \u2014especialmente has-link-color wp-elements-f48bc381127950c1748f9ed0e8bf9f6d\" style=\"font-size:22px\">Colesterol y enfermedad de Alzheimer&nbsp;<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Los resultados mostraron que el&nbsp;\u200b\u200b<strong>LDL-C&nbsp;es el principal factor lip\u00eddico asociado a la enfermedad<\/strong>.&nbsp;Los niveles de colesterol en el cerebro est\u00e1n estrictamente regulados por la funci\u00f3n fisiol\u00f3gica del cerebro, pero cada vez hay m\u00e1s pruebas que indican que el exceso de colesterol se acumula en el cerebro, donde puede provocar cambios patol\u00f3gicos asociados a la EA.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Sin embargo, los resultados no mostraron diferencias en los niveles s\u00e9ricos de HDL-C entre sujetos sanos y sujetos con asociaci\u00f3n significativa para la PAD; esto \u00faltimo, de acuerdo con estudios anteriores, podr\u00eda deberse a factores de confusi\u00f3n.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">La HTA se ha relacionado con atrofia cerebral y lesiones de la sustancia blanca y de los ovillos neurofibrilares. La&nbsp;<strong>ateroesclerosis podr\u00eda constituir un mecanismo etiol\u00f3gico compartido&nbsp;<\/strong>entre la enfermedad cardiovascular y la EA.&nbsp;<\/p>\n\n\n\n<div style=\"height:20px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h3 class=\"wp-block-heading has-black-color has-text-color has-link-color wp-elements-e6aaf759ea888862c8bc587e94b3f944\" style=\"font-size:22px\">Accidente cerebrovascular y enfermedad de Alzheimer&nbsp;<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Los&nbsp;<strong>microinfartos&nbsp;mostraron la asociaci\u00f3n m\u00e1s fuerte con la EA<\/strong>. Estas lesiones pueden alterar redes cognitivas clave y contribuir a la disfunci\u00f3n neurol\u00f3gica caracter\u00edstica de la EA.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u200b\u200b\u200bPromover h\u00e1bitos de vida saludables podr\u00eda reducir el riesgo de ictus y EA.&nbsp;<\/p>\n\n\n\n<div style=\"height:20px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h3 class=\"wp-block-heading has-black-color has-text-color has-link-color wp-elements-98e4cb05978951176c2f2f4e6cd77e0e\" style=\"font-size:22px\">Mecanismos fisiopatol\u00f3gicos&nbsp;<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Existen varias v\u00edas que pueden explicar c\u00f3mo los factores de riesgo cardiovascular contribuyen a la EA:&nbsp;&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>La HTA cr\u00f3nica y la ateroesclerosis pueden provocar hipoperfusi\u00f3n cerebral y da\u00f1os en la barrera hematoencef\u00e1lica, lo que facilita la acumulaci\u00f3n de amiloide\u202f\u03b2 y la patolog\u00eda tau.&nbsp;&nbsp;<\/li>\n\n\n\n<li>La dislipidemia, en particular el LDL-C elevado, puede afectar a la eliminaci\u00f3n de amiloide\u202f\u03b2 al alterar la funci\u00f3n de la apolipoprote\u00edna E y promover la&nbsp;neuroinflamaci\u00f3n.&nbsp;&nbsp;<\/li>\n\n\n\n<li>Los ACV y los&nbsp;microinfartos&nbsp;contribuyen a las lesiones de la sustancia blanca y a la atrofia cortical, lo que altera la conectividad neuronal y la funci\u00f3n cognitiva.&nbsp;<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Estudios recientes de neuroimagen tambi\u00e9n han demostrado que las personas con perfiles de riesgo cardiovascular presentan mayor atrofia cerebral, reducci\u00f3n del volumen del hipocampo y aumento de la carga amiloide.
